Have you tried reloading your UI?

I was seeing similar behavior and it seemed like reloading my UI resolved it (at least for a few runs).

There may be other ways to reload the UI but one way I think is to, in the 'AddOn List' area, re-enable and then re-disable an addon to enable the reload UI option.
