The other day I was doing some quests on my five man team on ground mounts in Dragonblight and tried to cross the ravine near OS, completely forgetting that I am in for quite a deep dive - and so were my toons.
I actually had to laugh about it since like so many things, I should have known better.

More facepalm moves than I care to remember were caused by those damned bridges and elevators in Thousand Needles. ARGH! http://ugh.scaredsheep.com/smileys/furious2.gif
Thousand needles was the first thing i thought about when I read the first posts. Can't recall how many times I fell from a high mountain. Thunder Bluff ain't much better. If an alt is stuck somewhere, I used to do a jump + forward on the alt, without turning broadcasting off ... = my 4 others jump from the mountain ... and when that happens 3x times in a row ...
Which reminds me to yesterday. I got me 5 flying mounts for the first time, so was all excited. Usually when I'm flying between towns I go grab a drink / smoke and when I come back my guys are just waiting for me at the airport. Now I was flying with my own mounts with auto run, got distracted and went for a smoke. To find my guys dead when I returned, as they crashed into some mountain and took a beating from a bunch of nasty mobs.
Or changing your keybindings ... what used to be the follow key is now the mount flyer key ... after taking off I wanna spam follow but hit the mount key ... 5x rip :/

I was running around in wester plaugeland doing quests and not notiching one of my toons diden't have one quest. So I did the quest moved on and suddently coulden't understand why the toon was lower on xp than the others.... notiched it diden't have the quest. I had to run back and do the same quest again = sucked!
This is very old, but it's from when I was solo player and wow was just out of beta... lots of years ago.
1 ) As a lowbie undead mage I found the Scarlet Monastery guards. The skull level looked like a target to kill, so I just attacked.
2 ) With my warlock in Deadmines got kicked fromtwo groups because I was "ninjaing" all the soul shards.
3 ) When the soul link bug was around I enslaved all the demons in the balsted lands and became "immortal". Proceeded to wipe an entire horde raid that was doing Kazzak and then finished him. Too bad the hordes got the loot instead of me :(
4 ) I kited Hogger to the Undercity just to have a guard kill him :(
5 ) When I finished the internal quest on my warlock I just ran to the crossroads and droppen an infernal just to have it turn on me 15 seconds later.
6 ) Summoned doomguard and infernal in goldshire and let them on rampage just to see a horde raid going to SW oneshot them :(
about two weeks ago i was changing my macros to make them more streamlined and to add a heal in my dps macro so i could just spam lightning bolt/heal without switching to another button and i didn't put a comma somewhere and the only macro i had working correctly was lava burst....
So i went to take a siege factory and there was one warrior there and i lava bursted him down to like nothing, like 200 hp left if that, and tried to finish him off but my lightning bolts weren't firing, and he wiped my whole group before i figured out what happened.
I thought he was hacking in some way because he took all 5 of my toons down lol
I've had various facepalms while multiboxing, but none of them amount to my biggest facepalm of all time:
