    You can multibox fairly good without any special use of mods. However there is a few reasons that I have modded my UI:

    - I use a 30" screen for my main. This will cover ALOT of your field of view. Therefor I have moved and resized my unit frames so I can get a better overview of my group for quick responding.
    - I have mod let me see castbars of my followers
    - I have mod that let me more easly target enemy targets while in chaotic fights (to pick out aoe classes etc)
    - I use Trinity bars since it gives me unlimited amount of macros, and extended size of each macro.
    - I use mods for relaying whispers from my followers to better keep track of them.
    - I use a bagmod for sorting and keeping track of my inventory (because the ingame system just suck )
    - I use auctioneer to cut my time spent at AH ALOT

    Guess there is a few more mods that I havent mentioned. Its perfectly viable to multibox without mods, but some mods make life alot easier IMO, and you save time spent on things you dont really want to be doing.

    I am going to look into mods like the one you said for easier picking out targets in pvp combat, that is one I could actually use.

    I use X-Perl because:
    - I want to see my party members' targets
    - I want to see what the focus is of each player (because I use that to attack)
    - I want to see the castbars of my partymembers, so I can see if there's something wrong
    - I can see the content of a raid. This is handy when I join a battleground with another multiboxer
    - some other minor reasons

    SpamMeNot is also great, because ... well, I'm sure you understand why :P

    well since august, my system has grown because of multiboxing... I started as 2 then 3 then 5 clients... I needed more screen real estate (@3) so a 24" monitor had to be bought, then for running 5 I upgraded my MB and memory (as well as the OS)

    so yes it evolves over time.

    I think the only addon that would cripple my gameplay would be the loss of trinity. losing a couple of hundred macros (across all 5 chars) for each team really would cause me some problems
