It's called [url='http://thottbot.com/i5462']Dartol's Rod of Transformation[/url]
You do the questline and on the last part when you get the rod just abandon the quest. Some people on thottbot say if you abandon the quest that the rod will disappear but it will not. Don't waste a spot for quests with it after you get it. You should be able to figure it out from the link I gave you above.

Yeah but to get "bear form", you have to roll the wrong faction. Not worth it.
