A quick google search mentions this sort of issue with some UI addons that can automatically adjust the resolution. I'd start there since the issue would probably present itself swapping between monitors with 2 different resolutions. You might also check for "gaming optimizers" that might be doing it. It seems more and more stuff is trying to "fix" the gaming experience by including things like Raptr, AMD's Gaming Evolved, etc. They could also be doing it.
