I would check if "Hold any Keystroke Actions while Hotkey is held" is checked. Look at the 2nd picture from the top, 2nd line in the bottom right pane, at the wiki entry for key maps at ISBoxer.com (
http://isboxer.com/wiki/Key_Maps). When you create a keymap with modifiers it must be checked or the modifier key will not go through to the game. On the other hand if you have both 1-= and alt+1-alt+= in the same mapped key it will not work. When ISBoxer hold the keystroke action it is holding down the key, in this case it is alt. So the 1-= become alt+1-alt+=. If hold keystroke is not set to on then the alt key is not being sent with the number key so the game isn't receiving the modifier. Hope this helps.
