How do you guys melee multibox? Curious if I'm doing it correctly

Like Kris1234 said, Interact with Target must be called every time your slaves need to move to follow. If you're doing PvP and you need to constantly spam IWT, you can add it to your DPS key using a Toggle. For example, if [ {Oem4} is your IWT key and 2 is your DPS key, you'd build the below hotkey -

     <Toggle>
          <SendLabel w2, w3, w4, w5>
               <Key Oem4>
     <Toggle>
          <SendLabel w1, w2, w3, w4, w5>
               <Key 2>

On the first press, it sends the IWT command to your slave windows and on the second press, it sends 2 to all windows.

Of course, spamming IWT will lead to your slaves constantly running around the target or running off into the distance, so you may need to be ready to combat that. Because of that, it's not something I would recommend for PVE.
