Try Riftblade for questing. Mine often outdamages my Pryomancer, just spamming Flamespear and Fiery Burst). Open with Frozen Armour on the Riftblade, which increases all fire damage the mob takes by 20% and snares it. Try using the Rogue Nightblade fire ranged attacks.

    Reaver/Pallie or Pallie/Warlord for instances.

Following builds I got from Zehn at FoH and work well (Pallie/Warlord from 17-50).

10 into Warlord for Death Grip is a great skill for casters.

Paladin/Warlord can be quite demanding to play. Reaver is not as good a tank, but they are easier to play and give solid AE dmg and AE threat. Basic Reaver/Pallie macro :

    cast disarming counterblow
    cast retaliation
    cast plague bringer
    cast soul sickness
    cast infestation
    cast pacifying strike

From Zantox, again on FoH :

You can flip soul sickness and infestation, but I found when tanking I want to make sure that soul sickness landed (and spread) to grab aggro on a pack. You can also switch pacifying strike out for something else you prefer to spam.

So for me, I start out Plague Bringer (macro) > Soul Sickness (macro) > Infestation (macro) > Necrotic Wounds (non-macro) > Blood Fever (non-macro) > pacifying strike (macro) then apply other debuffs, spam cleave or macro, always watching for my cool downs (I like to put my macro cooldowns in a side bar so I can watch where they are, even if it is just 1 button).
