At least one of the screenshots in the OP is not setup correctly, from what I've found.
Also, make sure you include the FTL assist step when creating the 2 step macro (unless you have an /assist command in your macro already)

Ok that answered my question number 1

it is 2=> ; => 2 => ; => 2 => ; spam, and the initial screen shot was incorrect

The toggle confused me quite a bit. I thought I have to toggle on the fly while I was spamming.



I still don't quite get number 2. But I guess it is just like click macro, which I never fully understand how it fires off.

i.e. I am half way through my lightning bolt, I spam 2=> ; like no tomorrow. When lightning bolt done, how do I know if the next spell is lava burst, not another lightning bolt.

Bollwerk is correct - the second screenshot is not correct. It should contain two steps that alternate "2" and ";", not one step sending both at once.
