    I'm not a DK expert, but I read on elitistjerks that if you have Frost Strike and are hit capped, its DPS is superior to Rune Strike. So use 1 or the other. Although, the below will still work with Rune Strike with a /cast !Rune Strike at the very end. You just won't see as many Frost Strikes.

    Using a /click macro, place /cast Frost Strike as the last action in the /click. This will cause you to fire off Frost Strike when not using any other abilities in your /click macro (the ones in the /click are probably more important anyway). Again, this will use Frost Strike when your other abilities are on cooldown or your Runes are refreshing.

    Example (AE mode, pulling manually with Icy Touch, Horn of Winter and following with Plague Strike, using Glyph of Disease and Glyph of Frost Strike):

    /click DPS1
    /click DPS2
    /click DPS3
    /click DPS4
    /cast Frost Strike

    DPS1 - /castsequence Pestilence,,,,,, ,,,,,, ,,,,,, ,,,,,, ,,,,,, ,,,,,, ,,,,,,
    DPS2 - /castsequence Blood Boil,,,,,, ,,,,,, ,,,,,, ,,,,,,
    DPS3 - /castsequence Death Strike,,,,,, ,,,,,, ,,,,,, ,,,,,,
    DPS4 - /castsequence Death Strike,,,,,, ,,,,,, ,,,,,, ,,,,,,

    4 presses / second. Each set of ,,,,,, = 1.5 seconds worth of presses.
    Initial Talented Icy Touch + Plague Strike + Horn of Winter = 35 RP
