Sometimes when everyone dc's at the same time the instance "owner" ends up being someone who wasn't the leader. If that happens you need to invite from that character.

Find which character isn't getting the message and invite your leader from it. You can then promote it to leader and it will be able to invite to the team to keep from being booted.
The next time this happens, I'll try to remember to wait and see if someone isn't getting the message. Thanks for the idea.
