However this thread could have promise if people start listing something totally "fail" they've done as a multiboxer (like running off a cliff with their main on accident and having all 4 clones follow them off).

Biggest fail for me was starting a quest that involved hopping on a plane - I have an addon that auto accepts and auto chooses options on all 4 clones, but before it could duplicate my "hop on the plane" click, my main's plane took off and the 4 clones turned and started following it - right off the ledge.
I ran off the cliff outside wyrmrest tower, if you run out and follow the road without looking you get a long fall with plenty of time to think about how much of an idiot you are before going splat.

I clicked the surfing on rockets quest without realizing I was going to take off this caused my followers to not only go off a huge cliff but to get scattered all over in the water and on the cliff surfaces in a way I eventually ended up spirit rezzing two of them after spending far too much time trying to get everyone back to their bodies.

Reoccuring Failure for me:

I use a dual-bar setup. Bar 1 PvE, Bar 2 PvP. I swap back and forth depending on instances vs. questing / ganking. Basically makes my healer and tank DPS.

I often zone into instances without changing my bars back to "PVE" then wonder when I wipe miserably. lol

I use separate bars for pve and pvp, the # of times I have been on the wrong bar smashing a button and wondering why I wasn't getting the expected result is embarrassingly high. The worst is my #3 bar is a utility bar with bandages, food, pots and that type of stuff forgetting to switch back and spamming away on those buttons does nothing except waste your stuff and get you killed ;)

My biggest fail.

One of my first 5 man setups was a client on each computer. Problem was, one of the systems was upstairs. Every time I had to accept a quest, or click a dialog box for a group, rez, death, etc I had to run upstairs. I started to hate escort quests as much as collection quests. More than once I was not able to accept the escort quest fast enough.

Several times ive spent an hour plus getting to the bottom of an instance to loot someones head only to get to the quest turn in and find one or more toons had either forgotten to loot it, or failed because their bags have been full for the last week and I didnt notice.

PvP. Many times at lower levels my entire 5 man group is killed by a high level toon with one aoe spell.
Missing picking up a quest item on one of your characters or one of them not getting credit for something and not realizing until you have run half way across the map to turn in a quest or hearthed, or going to do a quest and one of your characters didn't get it because their inventory was full.

How about a single rogue keeping your entire team busy messing with your main, that is embarrassing.
