Whenever I've had team members lose follow it was due to one of these:

1) The follower got stuck on an object

2) I had severe lag, (usually in Shattrath) so the main was moving, the follower was lagging.

3) My main (paladin) had crusader aura active, but I'd forgotten to /invite the rest of the team to the party, so they moved slower
