Has anyone gotten in trouble for just dual boxing? Are these permabans or warnings or suspensions at this point?

I would not chance my account by using ISboxer and instead would use something free and Open Source like https://wowopenbox.org/ or paid like https://joemultiboxer.com/ or go hardware boxing like this:

If my main boxing machine is powerful enough I likely would try to either do hardware boxing via a MultiSeat Desktop tool like Aster or via VMs before investing in 4 extra PCs. If you go the "Hardware input broadcasting boxing route" (regardless of whether you use PCs, VMs, or MDs) you will likely want a 'KM Sync' switch & the following is some info on the options and their gear compatibility:

There is also an article on cheap pc alternatives:

^ I think that's a great summary of where the state of multiboxing is at the moment.
There's lots of viable options - some slightly better than others depending what you want to do. I don't imagine non-broadcast software solutions like JMB or WOB will be popular in arena, but they're great for leveling and crafting. Any option will probably work for regular dungeons, but there is an edge to hardware broadcasting for TBC-heroics or retail Mythic+.

I've been lurking in the CS forums over on the WoW site for a few months now, and I will very often see people getting banned *supposedly* just for having isboxer installed on their pc. Another one I see people get banned for is joemultiboxer; players will often come in and complain that joemultiboxer says they dont use broadcasting (that's what's bannable when it comes to multiboxing, not multiboxing itself), but the standard reply those players get back is that it doesnt matter what the joemultiboxer website says... Basically, joemultiboxer is doing something that is triggering reports-- and with enough reports, someone is bound to come look into what you're doing.

You have to take all those posts with a grain of salt. It's a waste of time trying to derive anything from them.

From what I have seen it seems like Blizzard has been more active on policing Retail and tending to come down harder on the 2x4 farming setups in the open world and/or folk bothering other players regardless of what solutions you are using to multibox.
Just my subjective observations based upon what I have seen across many discords and forums and in all of those cases I only have limited one sided info... Also realize that most people don't fess up if they are/were doing anything fishy...
