I use Vista 64-Bit Ultimate.
Current issue, I'll move my mouse to my second monitor where my 2nd character is. Keyboard commands start going to him, however without moving the mouse back to the first screen/char or alt tabbing focus will randomly get switched back to the main thus causing keyboard commands to go to him.
Note: I am not running as an administrator as my mouse doesn't function properly in game if I do. Trying to test it now to see if that actually solves the focus switching issue however.

admin is usually required in order to do many things with separate process windows.
also make sure you have 'pass mouse clicks' UNCHECKED

So far running around Org I haven't been able to replicate the issue running as Admin. Pass mouse clicks isn't checked.
So now to the issue having w/ it as admin:
I can't use any mouse buttons I have assigned to keystrokes, in this case CTRL + Numpad 1-4
Nothing happens when I press the mouse buttons, which are currently bound to stances, attack, and ranged attack. Pressing the actually CTRL+NUM1 works. And all other mouse buttons work (1-5)
edit:
Playing with the mouse map, the box for mouse input will recognize buttons 1-5 but not the ones bound to keystrokes.
Edit:
Got it to work, running the mouse software as admin, then reseting up the buttons.

Well I just had the focus switch back to my main randomly once. So not sure if it's fully resolved. Make that Twice Now
Done it three times now: Main is outside BM in CoT and Alt is in Shatt. Trying to run my alt and it keeps switching to my main.

So far it's only been happening in Shatt, and quite often. Hasn't anywhere else.

Well it definitely happens quite often when a char is in Shatt, but it still happens elsewhere. Where focus randomly goes back to the main screen.
