I've added a 3rd monitor to my setup but ISBoxer only has 2 showing on the regions screen. Is that an ISBoxer issue, or am I doing something stupid on my end?

You'll have to delete that window layout and remake it [or just make a new one and keep the old one]. Old window layouts don't refresh with new hardware.
Also if you're not familiar with game profiles...
To get the best performance out of InnerSpace you'll want to make multiple profiles for your game. Right click on the tray icon and go to Configuration. Under the default profile you'll see it says 'Application Controlled'. If you're going to be utilizing all 3 monitors [that's what it sounds like] then you'll want to make 3 game profiles, each using their respective monitor. In that same drop down menu that has Application Controlled you'll see Primary/Secondary/Tertiary/Quaternary. Those are your monitors...not your video cards.
Once they're created in IS then restart ISBoxer and you should now have new profiles available under each character. Assigning characters the correct profile will help IS tell Windows which video card to run off of. If you keep it application controlled you'll be forced into using only the primary video card. Depending on your operating system will depend on your performance using a third monitor [or second graphics card]. Best results so far have been achieved on Windows 7 x64.
You can refresh the screens in the existing Window Layout by selecting "Current Computer" from the drop-down in the Regions panel, the one that says "<Pre-defined Computer>". The monitors attached to your PC are stored with the layout at the time of creation (hence "pre-defined", they were defined at that point -- it will go back to saying pre-defined computer after you save it), so that anyone else can load your configuration and see it exactly as it appears on your PC.
