Authenticator removal

I am trying to get my authenticator deactivated right now since i have lost my keys 8| I have the paper you have to fill out right here ('http://us.blizzard.com/support/article.xml?articleId=26361'). now my question is what sections do I have to fill out? im a little worried sending a "valid government issued document" :cursing:

driver's license will work or passport.

When they first allowed character transfers I forgot my secret question. So I had to fax a similar form with a copy of my driver's license to get it back to transfer a kid's toon off my account.

To complicate matters I had just moved and updated my wow info but not gotten a new DL yet.

It is not a big deal. They are confirming it is you before they free the account. Better that then anybody asking and you losing your account.

I use a G5 wired mouse.. i put my authenticator onto my mouse cable so I wouldn't loose it. I knew if it wasn't physically tied to my computer I would loose it too.

I sent my marriage license and it was rejected .... would there be a reason why?

I'd wager THOSE are for if you are trying to change your name on the account, adn you need to show the ID of the new name...

Send them something you could use to get into a bar... a drivers license, state ID, passport, etc.
