Howdy,
Was considering trying EQ2 but my friend convinced me to come check out LOTRO and so I am giving that a shot.
I have iSBoxer (on a trial right now) working to load up the two games and keyclone is passing keys (though I think if I can figure out how to do it I can stop using keyclone and just use isboxer for this function as well).
Hoping there are some LOTRO boxers lurking here who can shed some insight on how they have things setup.

Edit: Specifically, what you're doing for following, targetting etc. I know that there arent as many options as in WoW, just trying to see what I can do.

I have no info for you, but keep us updated with what you find. DDO and LotRO are both still on my list for eventual play.

I can not remember much, I played LotR for about 3 months when it first came out. I two boxed a Champion and Minstrel. I got them both to 50 with maxed tradeskills. I did not know about keyclone or innerspace at the time. I did it the old school "alt-tab" way.
What i do remember is you could make very limited macros at the time. I was able to follow and keep my leader targeted. It was easy to do because if your targeting a friendly target and you cast an offensive spell, it will assist your friendly target automatically.
I chose a champ, minstrel at the time for a two easy reasons. Champs can be tanks ( OT type ) and were ( might still be ) the best AE class in the game, they had melee attacks that laid waste to tons of mobs at a time. And the minst was the primary healer. SO it was an easy choice for two boxing.
I think they added two new classes since I played, Warden ( a tank type ) and rune master ( caster ). I know nothing about those atm.

Yeah I am trying out the different classes right now, so far I really dig this warden class (one of the new guys from Mines of Moria).
Probably going to hold off on the boxing til I get into the game a bit more and determine whether its something i want to invest in a couple more acccounts + innersphere subscription to box and whether or not i know what the hell im doing haha.

I'm dualboxing since a couple of days now, using hotkeynet and synergy (pc + laptop).
What I do for follow is: go in a fellowship, hit F2 (select fellow 1) and then I macro'ed ctrl + F for follow. I'm using 2 hunters now, because I find it very hard to go with 2 classes (above post: minstrel/champ for example). If anyone has some tips on how to arrange the skills so they match, feel free to give some info (like hit heal button on minstrel and do some attack on champ).
Targettign is pretty easy, just enable target skill forwarding on your alt. That way, when your alt has your main as a target, you just click the mob with your main and start using skills. The alt will automatically hit your main's target.

I 3-boxed it for awhile. But I was using 3 PCs with Multibox to broadcast keystrokes. The macros are quite limited, but using the alias system you can autofollow. And if you enable target forwarding you can just keep your main targeted at all times, and your healers/dps will automatically do their thing (e.g. a Minstrel's harmful spells will be forwarded to main's target unless it's a heal, etc.).
I played Guardian + Hunter + Minstrel (tank, dps, healer). I could pretty much take everything with that combo, even some of the epic chapters (dungeons). I think I got them to 30th level before I came back to WoW. It seemed like it was taking forever to level, and the quests started getting monotonous. Travel, while it seemed to take forever, was something awesome to behold with the artwork in the game. I highly recommend a Hunter in your party as they are the WoW equivalent of a Mage + Hunter (heavy dps, ports, group run buff, emergency escape port, etc.).
Minstrel is pretty much the option for heals. With a Guardian + Minstrel you can take on elites of equal level with no problem. Once I threw the Hunter into the mix for more DPS it was very nice. Like I said above, you can do the elite chapter instances with this group, but I had to overlevel them a bit (still recommend a full fellowship to be able to one-shot those), and they were getting much, much harder as I leveled up.
LOTRO is an awesome game, and if it wasn't for WoW I would still be playing it.

I'll give multibox a go, although hotkeynet is working pretty fine.
Did you use different keys to control different chars? For example ctrl key controls char 1, shift key controls char 2, no special key char 3?
